Build an index, not a folder list
A folder full of approvals is hard to review when each file has a different name and no connection to the payroll action. Give each decision a reference that links the request, source record, reviewer, approver, date, and resulting provider or employee action.
The index should show missing evidence explicitly. A blank approval field is more useful than a status that says “complete” because the file exists.
